Cause:
The form will not load properly if your Internet browser’s privacy settings are not allowing cookies to be set.

Solution:

Internet Epxlorer:

1. On the Help menu, click Contents and Index.

2. Click the Search tab.

3. Type privacy settings in the search box and then click List Topics.

4. Select the Topic you want to learn more about from the list and click Display.

Mozilla Firefox:

1. Press F1.
   Alternatively on the Help menu, click Help Contents.

2. From the list of Contents select Managing Cookies.

3. Select the Topic you want to learn more about.
